1. Make an appointment for a Sports Physical conducted by a licensed medical professional (i.e. your primary care doctor or urgent care/minute clinic). Use the (IUSD Physical Form). Parent section MUST be completed and hand signed/dated (MM/DD/YY). A doctor’s evaluation MUST include an office stamp and signature with date. Upload this document to your account.
2. Medical INSURANCE is REQUIRED for participation in Portola athletics. Options for insurance if not currently covered: Myers-Stevens.com
