Medical clearance is REQUIRED before trying out or participating in the PHS athletics program. Please make an appointment for a sports physical conducted by a licensed medical professional (i.e. your primary care doctor or urgent care/minute clinic), and UPLOAD the completed form dated after May 15th to your athleticclearance.com account. The IUSD Physical Form must be used and fully completed. The parent section must be hand signed/dated (MM/DD/YY). A doctor’s evaluation must include an office stamp and signature with date.
